This poll looks like it's meant for vanilla survival.
Skyblock modded will not work unless we switch to a skyblock pack.
(In my opinion, amplified would be dumb because we already barely get 60 fps in a normal world on modded, but yeah, I guess it *could* work.)
Ultra Hardcore will not work because of how easy it is to get killed by a glitch or by a boss.
Just make it a normal server.
As for pvp, I've never played on a public survival server really so you guys figure that out who know that stuff.

The survival server will most likely alternate between modded survival, modded creative, and normal survival.

Since Direworld20 has the Biomes o'plenty mod, I think instead of a normal world it should be a Biomes o'plenty world. It just adds a whole bunch of new interesting biomes and plants.

Yeah we don't need to vote on this, I'll use my mod server dictatorship to decide this is good.
